Why need 2 nameserver?
All dedicated servers should come with 3 ips. 2 are for the nameservers and the third is used as the servers main ip. If you want your website on a dedicated ip you will really want to ask for more ips. I'm rather surprised that the dns entries where not already setup for you. Even with a dedicated server any host should have this setup for you before assigning it to you.

Why need 2 nameserver?
While it may not make sense since both ns1 and ns2 are the same IP, you still need to set them both up. It is required, and is an industry standard. It is better to disobey the standard/requirement by only having 1 IP for both than to not set up the second ns record at all.

Have your DNS server setup beforehand on an unmanaged box? I don't think so. Different users do different things with their boxes and most don't want them cluttered up by provider-installed software. Even most control panels require you to have a clean slate before installing them, and only in that case would I expect to find a DNS server set up. Furthermore, the IP's have to be tethered to the domain at the registrar... Something the DC usually cannot do.
The only DNS servers you should have set up for you on a dedicated box are in resolv.conf, and those serve a completely different purpose.
